Abstract: Unhealthy diet and sedentarism are the main enemies of the liver, but Mediterranean Diet and moderate physical activity could prevent liver diseases, could promote MASH resolution and fibrosis regression. Changes in life-style requires a multidisciplinary team and holistic and personalised approach. Success rate in clinical practice is suboptimal and could be related to the interaction of the main drivers: amount of calories, quality of the diet, adherence to mediterranean diet, physical activity (frequency, type, duration and intensity), genetics (including PRS), microbiome and metabolomic profile. Life-style intervention as a lead-in phase could better stratify patients requiring drug therapy. Resmetiron, the first approved drug fro MASLD could be a great opportunity to better understand the complexity of this disease and to address liver fibrosis, a major concern in our daily clinical practice.
